How to make a FOI request

Your request should be in writing. This can be in the form of a letter, an email or the NHS Leeds FOI application form. This will need to be sent to:

Please note: from 1 April 2011 Leeds Community Healthcare (the former provider arm of NHS Leeds) became a NHS Trust in its own right, they will be responsible for provision of community healthcare services in Leeds. If you wish to make a request to the new trust you can do so by visiting their FOI page on their website (see related sites at the bottom of the page).

The information we need to be able to process your request

You should provide a name, an address for correspondence and the information you want. Please be as consise as possible about the information you want as this will help to find the information. Alternatively, you can print off and complete the FOI Request form (see related downloads).

Requesting Environmental Information held by NHS Leeds

The Environmental Information Regulations 2004 allows people to request access to environmental information held by public authorities such as NHS Leeds. The regulations allow access in a similar way to the FOI Act. The same type of details should be provided as for a FOI request (see above). The FOI Application form can also be used to make an environmental request.
