To ensure we provide the best possible services and our aim of improving the well being of the population of Leeds, NHS Leeds works closely with partners accross the city.
This can mean that personal information of patients can be shared between the appropriate agencies in order to provide co-ordinated care programmes for patients.
To share personal information, staff must be certain that conditions from legislation such as the Data Protection Act 1998 are met and that the duty of confidence of patient information is kept. This will ensure that information can be shared legally.
These conditions for sharing are set out in the Leeds Interagency Protocol for Sharing Information. This document acts as a guide for staff to follow when working with partner agencies. A number of agencies have signed up to the protocol, they are:
