NHS Continuing Healthcare is the name given to a package of services which is arranged and funded by the NHS for people outside hospital with ongoing health needs. You can get continuing healthcare in any setting, including your own home or in a care home as long as you meet the national criteria. More information is available in the NHS Leeds Continuing Care information booklet or you can download the national Continuing Healthcare information booklet.
Below is further information on the introduction of deadlines for assessments for eligibility for NHS continuing healthcare for cases during 1 April 2004 to 31 March 2012. Please note that legal representation will not affect the way in which the PCT deals with an individual’s request.

A new National Framework, implemented from 1st October 2007, provides an overall process for England covering NHS continuing healthcare and NHS funded nursing care. This framework was revised and a new version published in November 2012 - this will not be implemented until April 2013. Download your copy of the current National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care.
The Department of Health's website provides further information on the revised national framework for continuing care.
The Framework maps out a process for determining eligibility and introduces a set of national tools to support and improve consistency with regard to decision making. It also sets one band for NHS funded nursing care.
