The NHS has introduced the NHS Care Records Service (NHS CRS) throughout England and Wales. This is to improve the safety and quality of your care.
The purpose of the NHS Care Record Service is to allow information about you to be safely and securely accessed more quickly. Gradually, this will phase out difficult to access paper and film records.
There are two elements to your patient records:
• Summary Care Records (SCR) - held nationally, and rolling out in Leeds in 2009/10; and
• Detailed Care Records (DCR) - held locally.
