The Leeds Transformation Programme is a city-wide agreement between Health and Social Care partners to work together to deliver the challenges ahead, including increasing quality and innovation and productivity.
It is designed to bring key organisations together on this important task; to ensure their full engagement in identifying and delivering the most appropriate solutions to sustain quality while substantially reducing the overall cost in the Leeds health and social care economy by the end of 2014.
In parallel, the city is moving to a new model of health and social care as a result of the national reforms for the NHS and local authority, where we need to focus even further on:
The Programme is being led by NHS Leeds, which has the legal responsibility for improving health across the city. Therefore, the organisations listed below are key partners in the programme and have a seat on the Transformation Board which guides this work:
The Transformation Implementation Executive is chaired by Dr Simon Stockill, GP at Kirkstall Lane Medical Centre and a Board member for NHS Airedale, Bradford and Leeds.
To achieve our ambitious aims we have identified a number of key projects that will help us deliver the transformation to local health and social care services which we have set out to achieve.
